regarding checkboxes

Question

Am doing Hospital management project using asp.net web application. In that I have a appointments form, for this I have 91 checkboxes n dropdownlist for doctor's names n i have different timings n days like monday to sunday.I want 2 store this in a SQL server.when I run this form, if i clicked on any checkbox it tells true with that particular day n timings like 08:00 to 23:00 along with some Doctor name otherwise false.

   For this I have a idea that, create a table called DoctorID, in that we have day1,day2.......,if we checked means it shows true else false .Also, for DoctorID timings. But I don't know how to implement n code them.please send with detail steps.

            Please send the code in javascript for this as fast as possible. Its very Urgent.
