MVC 4 Multiple choice Question application

Hi

 

I am new to MVC 4, i want to develop a servey module using MVC 4,

 

Like single question having multiple answers EX: Agree,Disagree

and every question having same set of choices, how can achieve this using mvc4 choices should be radio buttons .
