how to create a repot of a sigle record using vb.net

Im building a project for registration in hospital, i want that when a new patient registered i create a print a slip that contain some fields in it like Patient_id,Patient Name.Cashier, Date,...that they are certainly fields of my tables in database, how to create a button to create this report and print it.
Please guide me.
